    Registration closes April 30th at 11:59 PM, late registrations will be accepted at the discretion of the parent rep/coaches if there is space remaining in that age group, and a $25 fee will be charged.

    Durham Summer Swim League Waiver and Release of Liability

    AGREEMENT FOR PARTICIPATION IN DURHAM SUMMER SWIM LEAGUE 

    In consideration of being allowed to participate in any way in the Durham Summer Swim League, the undersigned acknowledges, appreciates, and agrees that: 

    1. The risk of injury from the activities involved in this program is significant, including the potential for paralysis and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injuries does exist; and, 

    2. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or other, and assume full responsibility for my participation; and, 

    3. I willingly agree to comply with the stated and customary terms and conditions for participation. If however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and, 

    4. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS DURHAM SUMMER SWIM LEAGUE, their officers, officials, agents and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and, if applicable, owners and lessors of premises used to conduct the event ("Releasees"), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE. I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT. 

    FOR PARTICIPANTS OF MINOR AGE (UNDER 18 AT TIME OF CHAMPIONSHIP) This is to certify that I, as parent/guardian with legal responsibility for this participant, do consent and agree to his/her release as provided above, of all Releasees, and, for myself, my heirs, assigns, and next of kin, I release and agree to indemnify the Releasees from any and all liabilities incident to my minor child's involvement or participation in those programs as provided above, EVEN IF ARISING FROM THEIR OWN NEGLIGENCE.

    Digital Media Consent Form

    Photos and videos are taken at our meets and practices. Please acknowledge your consent for your athlete(s) and their family/supporters at our meets & practices to have their photo and/or video taken. Photos are put into albums and published on our team website. I also give consent for these photos to be used in team communication, the team website, and the team social media pages.

    COVID-19 Precautions

    Due to the ongoing pandemic, there will be COVID-19 precautions for this season based on state, local, and DSSL requirements. While it is not possible to know what requirements will be in place come mid-May, please acknowledge your family's adherence to any and all COVID precautions put into place at any time during the swim season. Failure to comply may result in you, other members of your family, and/or your swimmer(s) being asked to leave an event. Repeat violations may result in your family's removal from the team. It is anticipated that capacity limits will be in place for practices, meets, and other events. This means that parents will most likely not be allowed on deck during our practice times or at meets, regardless of the age of your swimmers. Safety is our utmost priority at all times, especially this coming season!

    By checking the box below, I agree to uphold all safety precautions put in place at any time by the Garrett Farms swim team and DSSL. I agree to adhere to any rules in relation to the swim team/DSSL as may be announced at a later date prior to or during the swim season to protect not only myself / my family / my swimmer(s), but also the GF community as a whole.
